Abstract

Type-specific serologic tests for human herpes simplex virus (HSV) are critically important for sexually transmitted disease evaluation. We compared the LIAISON® HSV-1 and HSV-2 Type Specific assays relative to an established commercial ELISA. The overall agreement of the chemiluminescence immunoassay versus the ELISA assay was 99.6% (HSV-1) and 100% (HSV-2). The LIAISON® methodology has several advantages.

All incubations, washing steps, and assessment of chemiluminescence are performed in a closed system. Results are expressed as an index value; indices <0.90 are considered negative, 0.90–1.09 equivocal, and ≥1.10 positive (Maters et al. 2012). For samples within a given inhibition assay result category (TP, FP, IND), the proportions of samples with a screening index >3.50 in the two HSV-2 IgG screening assays (HSLT versus Liaison) were compared using Fisher's exact test.
CDC guidelines recommend confirmatory testing of sera with low-positive indices (1.10–3.50) in the HerpeSelect® (HSLT) HSV-2 IgG screening assay. To determine if this recommendation is adequate for our patient population, we reviewed HSLT HSV-2 IgG screening indices for 262 screen-positive sera (index >1.10) tested in our confirmatory assay, which assesses inhibition of binding to recombinant gG2 by HSV-1- and HSV-2-infected cell lysates. To determine how the recommendation affects other screening assays, we tested these samples in the Liaison® HSV-2 IgG assay. Of 124 false-positive sera, 20% and 39% had an index >3.50 in the HSLT and Liaison screening assays, respectively. In both assays, 51% of 63 indeterminate sera (inhibition by HSV-1 lysate) had indices >3.50. Similarly, ≥75% of 75 true-positive samples exhibited indices >3.50 in both assays. Thus, confirmatory testing only of sera with low-positive HSV-2 IgG indices misses some false-positive and indeterminate samples, leading to misdiagnosis of HSV-2 infection.

La lesión por VHS continúa siendo la causa más frecuente de úlcera vulvar entre la población sexualmente activa, y su incidencia aumenta cada año. En esta revisión resumiremos la microbiología del virus, la patogenia y la infección en genitales, las manifestaciones clínicas para su correcta identificación, las diferentes técnicas diagnósticas de laboratorio y la elección del correcto tratamiento según sea primera infección, recurrencia o casos especiales. Finalmente, se discute un análisis de costes de la enfermedad por VHS.
Genital herpes is a sexually transmitted disease caused by herpes simplex virus type 1 (HSV-1) and type 2 (HSV-2) belonging to the alphaherpesvirus family, that includes the varicella zoster virus. HSV infection continues to be the most common cause of vulvar ulcers among the sexually active population. Its incidence increases every year. This review summarises the microbiology of the virus, pathogenesis and infection in genitalia, clinical manifestations and correct identification, the different laboratory diagnostic methods, and choice of the correct treatment according to the first infection, recurrence or special cases. Finally, the cost of routine herpes simplex virus infection is analysed.
Seroepidemiology for measles among elementary school children in Northern Taiwan

The difference in the seropositivity rates between the tests by Liaison CLIA and NovaLisa EIA was statistically significant. CLIAs have been reported to provide equal, even greater, sensitivity and specificity in serologically diagnosing a variety of infectious diseases when compared to traditional EIAs.6–11 Liaison measles IgG is a new semiqualitative, fully automated method for determination of measles-speciﬁc IgG, and the sensitivity and specificity provided by the manufacturer were 94.7% and 97.4%, respectively.
Despite the high vaccination coverage in Taiwan, sporadic cases or small cluster of measles still occur every year. Nevertheless, few studies provided information about the serostatus for measles in recent years. This cross-sectional survey aimed to establish the seroepidemiologic data of measles among elementary school children in New Taipei City, Taiwan during 2012–2013.
A multistage stratified sampling design using 14 variables was employed to obtain samples. All selected schoolchildren were bled for the serologic tests of measles by both chemiluminescence immunoassay (CLIA) and enzyme immunoassay (EIA).
A total of 856 schoolchildren from 14 schools were recruited in this study. Among these individuals, the overall seropositive rates for measles were 82.24% and 92.17% by the Liaison and NovaLisa assays, respectively. For schoolchildren in each grade, the seropositive rates were > 90% for Grade 1, and then decreased gradually to 70–80% for Grade 6 (p < 0.001 for both methods). A decay of measles-specific immunoglobulin G titers was also observed with the maximum drop between Grade 1 and Grade 2 (declining trend of p < 0.001 for both).
Although the seropositive rate of measles was > 90% for Grade 1, the rate for Grade 6 was only in the range of 70–80%, which may be a challenge to prevent outbreaks of measles in the future and should be monitored meticulously.
